With Addo Forms, you can create an electronic form, which is either sent by e-mail to a recipient or made available via a link. With Forms, you have the option to collect information, get signatures, send out questionnaires and have simple questions answered in the same process.
Addo Forms is suitable for many smart purposes - you can e.g. make employment contracts in Forms, and obtain all necessary information in just one flow. Where can I set up my template?
You will find Addo Forms in your settings.
When you access the "Forms" tab, you can click "Create new form" where you will find several options for setting up your Form.
For each option, you can set requirements for whether the type is "Mandatory". This gives the recipient no choice but to reply before the Form can be approved.
You also get the option to select "Prefill when sending", which means that you as the sender must fill in the information.
Options when setting up the template
Title - You can provide titles for sections or questions.
Text - You can use a text field to specify what the form is about or provide other sorts of information.
Single-line text answer - You can ask a question here that requires a short answer from the recipient.
Multi-line text answer - You can ask a question here, where your recipient will have the opportunity to give a longer answer.
Radio buttons - Here you can give your recipient several answer options when answering a question, but only a single answer can be selected. If "Has dependents" is checked and the recipient selects a reply, you as the sender can "Add dependent component". This means that the recipient must elaborate on his/her answer, where you again get the same options as under "Create ...". In other words, the recipient with this feature may be required to answer additional questions or upload a file for that answer.
Checkboxes - You can allow your recipient to tick several answers to a question.
File upload - You can require the recipient to upload a file that you as the sender will subsequently be able to download.
Image - You, as the sender, can upload an image for your Form. Please note the difference between File upload and image, as File upload would be the choice if you need the receiver to attach a photo or file, and image upload would be used if the sender needs to upload an image.
Date - You can allow your recipient to select a date in a calendar.
At the bottom of Addo Forms you will be able to "Enable public link". You can read much more about this here.
